Illinois State Board of Education (ISBE) started requiring input surveys several years ago. Farmington Central CUSD#265 has traditionally utilized the data within our Organizational Health Index (OHI). ISBE has begun using 5Essentials system to collect survey data. The University of Chicago Consortium on Chicago School Research has identified components that are deemed to be critical for school success.
